This document affects your legal rights. By signing this waiver, you acknowledge and accept the risks associated with martial arts training and waive certain legal rights.

I, {name}, hereby submit my application for participation in Brazilian Jiu Jitsu training and related activities provided by Point Break BJJ Pty Ltd (ABN 53 686 309 802), trading as Point Break Brazilian Jiu Jitsu (hereafter referred to as 'Point Break').

I acknowledge that participation in Brazilian Jiu Jitsu involves significant physical exertion and risk. I assume full responsibility for any and all injuries, losses, or damages that may occur while participating in or observing classes, seminars, competitions, or other activities related to Point Break. I understand that serious injury or even death is a real possibility due to the nature of the sport.

I further acknowledge that if I am injured, the consequences may include, but are not limited to, disfigurement, permanent disability, or loss of income. These risks may result from my own actions or inactions, the actions or inactions of others (including instructors and training partners), the condition of the mats or premises, or other unforeseen factors.

By signing this agreement, I knowingly and voluntarily assume all such risks and hereby waive any and all claims I, or anyone acting on my behalf or through me, may have against:

- Point Break BJJ Pty Ltd (ABN 53 686 309 802)

- Its directors, employees, contractors, and representatives

- Point Break BJJ Pty Ltd

- All instructors, staff, volunteers, training partners, participants, and non-participants

- The Australian Federation of Brazilian Jiu Jitsu Inc

- Any officials, sponsors, or affiliates

I consent to receive medical treatment, including first aid, by authorized personnel on the premises in the event of injury or medical emergency.

I affirm that I am in good health, and that I will not participate while under the influence of drugs or alcohol. I confirm that I have not been coerced or misled by any staff or representative of Point Break regarding my fitness or ability to participate. I acknowledge that I have read, understood, and agree to be bound by all provisions of this agreement, and that I am legally competent to do so.
